Job Description:
As a Personal Care Manager at Citraverse, you will play a pivotal role in providing personalized care and companionship to elderly individuals in the comfort of their homes.
Responsibilities:
Provide personal care assistance to elderly clients, including bathing, grooming, dressing, Diaper change and toilet needs.
Assist with mobility, transfers, and ambulation as needed.
Administer medication according to prescribed schedules and instructions.
Assist with feeding if required.
Engage clients in meaningful activities and companionship to promote social interaction and mental stimulation. (Gardening, Reading, Music, Stimulation games etc)
Maintain a safe, clean and hygienic environment for clients, ensuring their comfort and well-being at all times.
Monitor and report any changes in clients' health or behavior to the designated care coordinator.
Accompanying clients on outings and medical appointments.
Engaging clients in conversing and walks.
Massage therapy and assist clients in physical therapy exercises.
Collaborate effectively with healthcare professionals, family members, and other caregivers to ensure coordinated care delivery.
Update daily care plan in the platform.
Identify red flags and inform the healthcare provider/family.
